Our Approach
Our interdisciplinary team conducts comprehensive medication reviews, monitors clinical outcomes, provides patient education, and coordinates directly with prescribing physicians. We leverage telehealth platforms to ensure continuous access to care — reducing emergency department visits and hospitalizations.

Programs & Services Under This Solution
Chronic Care Management (CCM)
CMS-recognized CCM services providing at least 20 minutes of non-face-to-face clinical staff time per calendar month for patients with two or more chronic conditions. Includes care plan development, coordination, and ongoing monitoring.
Diabetes Support Program
Comprehensive pharmacist-led diabetes management including blood glucose monitoring guidance, insulin initiation support, A1C tracking, self-management education, and lifestyle counseling for Type 1 and Type 2 diabetes patients.
Hypertension & Heart Disease Program
Evidence-based protocol for managing hypertension, heart failure, and coronary artery disease — including home blood pressure monitoring support, medication optimization, dietary guidance, and regular clinical check-ins.
Medication Therapy Management (MTM)
Comprehensive Medication Review (CMR) and Targeted Medication Review (TMR) services to identify drug interactions, address adherence barriers, and optimize therapeutic outcomes for complex patients.
COPD & Asthma Management
Respiratory disease management including inhaler technique training, action plan development, exacerbation prevention strategies, and coordination with pulmonologists to improve lung function and reduce hospital admissions.
Telehealth Disease Management
Remote patient monitoring and telehealth-based disease management services enabling patients to receive high-quality chronic care from the comfort of their homes — with regular virtual check-ins and real-time data review.
