What is a Practice Support Network (PSN)?

When you join our Practice Support network you have access to a team of clinical pharmacists that may be integrated into your practice location or consulted on demand for support. As a member of our Practice Support Network (PSN), you are able to customize your service request or select from a range of services offered that best suits the needs of your practice.

What type of experience do the PSN Pharmacists have?

Our Practice Support Network is comprised of highly trained and experienced clinical pharmacist with a wealth of knowledge in managing chronic diseases and complex health issues. Our Pharmacists are uniquely qualified medication experts with advanced training to support the individualized needs of your practice and patient population. Our team of pharmacist can offer an advantage to your practice as an integral member of an interdisciplinary team to help achieve desired patient health outcomes.

Our pharmacist hold doctorate degrees in Pharmacy (PharmD) and most have completed general and specialized pharmacy residencies in pharmacy practice, ambulatory care and other specialties and hold pharmaceutical board certifications in geriatrics, pharmacotherapy, ambulatory care, cardiology, nutritional support and psychiatric pharmacy.

How does it work and how do I join the Practice Support Network (PSN)

  • Receive complimentary practice evaluation & recommendation for Pharmacist integration
  • Set agreement defining Pharmacist role (based on state’s board of pharmacy law)
  • Set agreement defining pay scale and scope of service
  • Matched with a Clinical Pharmacist that meets the needs of your practice
  • Set expectations for your team and patients and effect on practice workflow
  • Obtain resources needed by pharmacist for continued success

Does my practice need to join the Practice Support Network (PSN) to be matched with a Pharmacist?

Practices outside network can still receive access to clinical pharmacy services or customized services offered in network on demand by completing a patient referral request form. Practices requesting ongoing services outside the practice support network (PSN) will need to complete a request for information form and establish a scope of service agreement.
