Prime Care Health Solutions is a healthcare services organization founded in 2018 by a Pharmacy Doctor (PharmD.) and a passionate patient advocate that saw the benefit and need for more Pharmacists in direct patient care clinical roles to address healthcare disparities in the management and prevention of chronic disease, promotion of wellness and reduction in healthcare costs.
Prime Care Health Solutions specializes in designing and delivering superior healthcare programs & services and innovative clinical pharmacy programs to individuals, provider practices, health plans, employers, health care facilities and organizations. We promote exceptional-performance based clinical outcomes, cost savings and positive patient outcomes.

Prime Care Health Solution’s multi-disciplinary team approach involves our pharmacists partnering with other members of the healthcare team to provide clinical pharmacy services in disease state management, prevention, wellness and transition of care services.
Our pharmacists are uniquely qualified medication experts with a wealth of knowledge in managing chronic diseases and complex health issues. Many of our pharmacy doctors have completed general and specialized pharmacy residencies in pharmacy practice, ambulatory care and other specialties and hold pharmaceutical board certifications in geriatrics, pharmaco-therapy, ambulatory care, cardiology, nutritional support and psychiatric pharmacy.
Our team of pharmacy doctors can offer an advantage to any health care facility or organizations by working as part of an interdisciplinary team to help achieve desired health outcomes.
Our wellness practitioners, health coaches and health educators collaborate with other healthcare professionals to provide in person or tele-health (video and phone) coaching, classes, workshops and educational health &wellness services to participants on healthy lifestyle choices to improve overall health.
We have an impactful approach to care management and administrative health care services that involves integration of our pharmacist experts as a vital member of the care team that closely collaborates with prescribers, nurses and ancillary specialists to achieve positive outcomes.
